My daughter is 4 years old, she has ringworm on her head, is it adviceable to use relaxer to treat it or antifungal cream?

You will need to see a doctor as the child needs antifungal medications both oral and topical for treatment. Treatment may last up to 2 - 6 weeks depending on the medications prescribed for complete resolution

Can I add relaxer (perm), to my two-year-old daughter's hair, is it safe? Thanks for your helpful answers.

No ma it is not. Please leave the baby's hair alone....If it is difficult to comb there are disentangling shampoo or better still review your intended relaxer manufacturer's guide to know what age range it is appropriate for.
