If your baby is on exclusive breastfeeding, it's okay. Babies on EBF do not need to poo daily, they can stay up to 2weeks without having to poo. Sometimes, they can even poo up to 12 times in a day. As long as the stools are normal. Nothing to be worried about.
Your 4 months old is expected to be fed breast milk only. No orange, water etc till after 6 months.
The child should be seen by the Paediatric Surgeon and a Paediatric gastroenterologist for further evaluation for causes of chronic constipation. And you should also go back for further appointments and follow up with the specialists.

You can keep them in the car seat put in an upright position if you are too tired or busy to carry them that long...others can help too.....sometimes that may not work and ....but we will do that only if it so bad and not allowing the baby to gain weight....please see your paediatricians for further management at that point
