As regards the sleeping pattern, it has nothing to do with you not giving pap. Baby's sleeping pattern is usually not like yours in the first few months...so it is perfectly normal for them to sleep more in the day and be awake at night! Don't worry this too will come to pass :) just try and sleep when the baby is sleeping so you can do what we called the baby's vigil well.....after a while the baby will change to your pattern
A 4 months old baby should be on exclusive breastfeeding alone. Not water, not anything. If for health or any other reasons you can not breastfeed exclusively, kindly give age-appropriate milk/formula and hold till 6 months to introduce complementary feeding.
