DEPENDING ON WHAT YOU MEANT BY YELLOW URINE and at what point? The normal Urine colour is AMBER - close to water colour. There is no specific number of times a child should urinate the most important is the quantity of urine passed in the day. It depends on how much fluid or breastfeeding you do at night. Since it is a boy you can take off the diaper and watch him after breastfeeding/feeding well, if no urine or very small quantity in 4 hours, take him to the hospital so the urine can be measured accurately to take a decision. The best way to know is to breastfeed well and then check if he really passes urine don't rely on diaper feel some are hyperdry, check the urine stream with your eyes.

Stale urine should naturally smell. It depends on the Type of smell. There is nothing wrong with the urine smelling except it is a specific kind of smell that may suggest a disease. Sometimes the smell is a reflection of the child's intake. For example children on antibiotics may have the urine taking on the smell of the particular antibiotics the child is taking. If you think the smell is so bad you may take her to see a paediatrician

It is not good, a urinalysis test will need to be conducted to confirm whether their is sugar in the child's urine.

I'm not sure what you meant by a thin layer formed on top. Then what kind of smell did you mean? It is better to see a doctor for proper evaluation if you are concerned

Change diaper frequently when wet...not only when there is a poo and also Apply diaper cream - vaseline, sudocream, shea butter, zinc oxide cream are fine...Not powder pls....it must be something that creates barrier between skin and urine....powder will be formed into paste by urine and further irritates the skin..
